Diabetes Management Center

Riverview Medical Center knows how important self-management of diabetes is to a growing number of people. That's why our Diabetes Management Center offers a comprehensive and individualized program, focusing on wellness and prevention. No matter what type of diabetes you have, we work with you to develop specific lifestyle changes that will allow you to live well with diabetes.

The Diabetes Management team consists of knowledgeable and experienced diabetes educators who are committed to your care. You can be sure you're receiving the best care, since our Diabetes Management Program was granted national recognition from the American Diabetes Association for our high quality education.

What to Expect for Your First Visit
The Diabetes Management team works with you and your physician to help you manage your diabetes safely and effectively. A prescription from your physician is required. Your first visit will be a comprehensive one-on-one assessment to determine your needs and begin the education process. At the end of this session, you will have your own personal meal plan, which you and our dietitian have created together.

Please call us at 732-530-2555 to set up your initial two hour appointment and address any questions that you may have. Once you have scheduled your appointment, look over the Welcome Letter, you will need to fill out the Diabetes Management Program Needs Assessment and bring it with you, and you can also track your blood glucose by using our Blood Glucose Log Sheet. Please remember a physician prescription is required for this service.

Class Programs at Convenient Times
Riverview's Diabetes Management Program offers a series of classes during days and evenings. These classes cover:

  • Diabetes disease process and treatment options
  • Nutritional management
  • Incorporating physical activity into your lifestyle
  • Utilizing medications (if applicable)
  • Monitoring blood glucose and pattern management
  • Preventing, detecting, and treating acute complications
  • Risk reduction, detection, and treatment for chronic complications
  • Goal setting and problem solving
  • Integrating psychosocial adjustment (coping skills)

Help for Gestational Diabetes, Too
Riverview's Diabetes Management also offers vitally important pre-conception and gestational diabetes education for women who are planning a family.
