To become one of these participants and start reaping the many benefits of our program, all you need is a prescription from your physician. Once you have been referred to us, we work closely with both you and your doctor to manage your diabetes safely and effectively.
Types of Diabetes
- Type 1 Diabetes: Type 1 diabetes is the result of the body’s failure to produce insulin. This form of the disease is sometimes referred to as insulin-dependent diabetes because insulin must be supplied to the body daily. It has also traditionally been called juvenile diabetes because it often, though not always, develops during childhood or adolescence. Approximately 5 to 10 percent of diabetics have type 1 diabetes.
- Type 2 Diabetes: Type 2 diabetes results from insulin resistance – when the body is unable to properly use insulin – combined with an insulin deficiency. Not only is this the most common form of diabetes, but a large number of people with type 2 diabetes are not aware of their condition because they have no noticeable symptoms.
- Gestational Diabetes: Gestational diabetes occurs exclusively in expectant mothers, usually during later pregnancy. This form of diabetes affects about 4 percent of all pregnant women. Women who have gestational diabetes are at increased risk to develop type 2 diabetes later in life. However, research has shown that this risk may decrease in women who are physically active and maintain a healthy weight.
- Pre-Diabetes: Pre-diabetes occurs when blood glucose levels are elevated above normal but are not high enough for a diagnosis of type 2 diabetes. There are 41 million Americans with pre-diabetes. This is in addition to the 18.2 million with diabetes.
What to Expect for Your First Visit
During your first visit, we’ll conduct a thorough one-on-one assessment, at which time your individual needs will be determined and your education process begun. At the end of this initial session, you’ll receive your own personal meal plan, which you and your dietitian will have created together. Team members will also show you how to test your blood sugar using the testing meter of your choice and how to properly maintain your testing machine.
Please call us at 732-530-2555 to set up your initial two hour appointment and address any questions that you may have. Once you have scheduled your appointment, look over the Welcome Letter, you will need to fill out the Diabetes Management Program Needs Assessment and bring it with you, and you can also track your blood glucose by using our Blood Glucose Log Sheet. Please remember a physician prescription is required for this service.
Class Programs at Convenient Times
Riverview's Diabetes Management Program offers a series of classes during days and evenings. These classes cover:
- Diabetes disease process and treatment options
- Nutritional management
- Incorporating physical activity into your lifestyle
- Utilizing medications (if applicable)
- Monitoring blood glucose and pattern management
- Preventing, detecting, and treating acute complications
- Risk reduction, detection, and treatment for chronic complications
- Goal setting and problem solving
- Integrating psychosocial adjustment (coping skills)
