George and Vita Kolber Family Health Center
The family health center offers coordinated, comprehensive, primary health care for adults, children, podiatry, prenatal and women’s health care services. The goal is to control disease, prevent further illness, and promote wellness and self-care from a team of health professionals, which includes nurses, physicians, nutritionists, and financial counselors.
Who We Treat
Newborns to adults can be treated at the health center. Services are provided to patients who participate in a broad range of financial reimbursement plans, including Medicaid patients who are enrolled in specific HMOs. All prospective patients are required to go through a financial screening process before using health center services.
Referred to Physicians for Follow-Up Care
If you require follow-up subspecialty care, we will refer you to physicians who are part of a specific HMO. All other health center patients are referred to a private physician.
